On Wednesday morning the CNESST, Quebec’s workplace health and safety standards commission, released its report on the deadly scaffolding collapse at the Domtar mill in Windsor last fall to the public. Based on detailed investigation, the report attributes the incident to two main causes; several shortcomings in the design of the scaffolding set up, and failure of at least one piece of the structure due to a weight overload, leading to collapse.
“This kind of accident rarely has a single cause,” said Maryline Boulianne, inspector with the CNESST, pointing out that the incident took place during routine maintenance on the factory’s ten storey digester tank, work that is complex but needed on a regular basis to maintain the large piece of equipment.
Boulianne reminded those present at Thursday’s presentation of the report that the situation last October required a very particular response and created some complications for the completion of her report.
